You can prepare three your own portraits with names: Small, Medium and Fulllength. All these images must be saved in PNG format with resolutions:
Small – width: 185px, height: 242px;
Medium – width: 330px, height: 432px;
Fulllength – width: 692px, height: 1024px;
The gameflow looks like:
1. Start a New game.
2. Choose “Create a new character”.
3. Click on icon “Custom portrait”.
4. Click on button “Open portraits folder”. (It works for Windows and Mac)
5. Replace portraits in opened folder or change it in your graphic redactor.
6. Click “Refresh portraits”.
7. Enjoy your own portraits.
Also it works for custom companions.

I seem to have a problem that doesn’t seem to be able to be fixed, i’m trying to add new portraits to my game(for specific characters i’m trying to use. I was able to add the portraits with little difficulty, but when I try to play the game with the portraits I added, my game stalls or something, i can’t exit the game or close it, and i can only do two things to restart, i either have to hit the reset on my tower, or hit the windows button and choose to restart. I’ve tried it many times, and i can’t seem to fix it. though if i only use the ones that are already in the game it’s fine, i don’t know what else to try, my video card drivers are up to date, i don’t seem to have anything else to try, a video said it was easy to do but i have yet to find that at all, I have set the Small Medium & Fulllength portraits for the ones i want, i hope there is something i can do to use the protraits, i have a portrait for every character i want to make in the game, but can’t get my computer or game to allow them or something, i don’t know how else to explain my problem, i hope someone can help me figure this out
I had the same problem. Try switching graphics display mode from “fullscreen” to “fullscreen windowed borderless” (or something like that ; my game is not in english). It worked for me, I can now alt-tab without crashing my game 🙂
