Exhaust 20 cards in a single combat.
Aside from doing Purity with a decksize larger than 23, this achievement counts any Exhaust cards.
Most importantly, it counts Shivs, so play as The Silent and get as many Shiv generators as possible. Endless Agony also speeds this up nicely.
